为树立在实习期间的安全意识，避免不安全事故发生，保证实习工作顺利进行，特签订以下安全责任书：

1．遵守国家法律、社会公德和校纪校规，遵守实习纪律，团结互助；

2．遵守交通法规，注意铁路、公路、水路交通安全。除分散实习外，集体出发、集中返校，沿途不逗留、游玩；

3．严守实习单位的安全保卫规定、劳动纪律；在操作前自觉接受安全技术培训，作业中严格遵守操作规程，未经允许不擅自调换工种和设备，不擅自动用与实习教学无关的设备、仪器和车辆等，不在实习场所嬉戏、打闹；

4．服从带队教师（指导教师）的管理，听从带队教师（指导教师）的指挥；尊重实习单位的领导、职工和指导教师；

5．严格遵守请销假制度。做到按时出勤；特殊情况外出，向实习带队教师（指导教师）请假，归队后及时向实习带队教师（指导教师）销假；

6．不违章用电、用火，不去游泳池、江、河、湖、海等地游泳或洗澡，不做其它任何影响安全的事情；不去网吧、歌厅等场所；

7．实习期间不外宿、酗酒、寻衅闹事、打架斗殴等，不在实习宿舍内留宿他人；

8．严格遵守实习期间作息时间，不擅自离开实习地点到外地游玩；

9．努力提高安全防范意识，提高自我保护能力；注意自身的人身和财物安全，特别是交通和饮食安全；

10．遵守实习地的地方法规，尊重当地群众的风俗习惯和宗教信仰，爱护一草一木，避免与群众发生冲突，做到文明礼貌，维护学校声誉。

In order to establish safety awareness during the internship, to avoid unsafe accidents and to ensure the smooth implementation of the internship, I hereby sign the following safety responsibility letter:

1. I will abide by national laws, social ethics and school rules and regulations, abide by the discipline of internship, and unite with other classmates.

2. I will abide by traffic regulations and pay attention to railroad, highway and waterway traffic safety. Except for decentralized internship, I will definitely depart and return to school collectively with other classmates, and will not stay or play along the way.

3. I will strictly abide by the safety and security regulations and labor discipline of the internship unit; I will consciously accept safety technical training before operation, strictly abide by the operating procedures during operation, not to change jobs and equipment without permission, not to use equipment, instruments and vehicles unrelated to the internship teaching without permission, not to play and play in the internship site.

7. I promise not to stay out during the internship, not to drink too much, not to pick quarrels, not to fight, etc., and not to stay with unrelated people in the internship dormitory;

9. I will try to improve my awareness of safety and self-protection; I willpay attention to my personal and property safety, especially traffic and food safety.
