实习单位考核意见：要求实习单位指派专人对学生具体指导时，由实习单位的指导人员填写，没有配备实习单位专门指导人员的，本栏不填写；
指导教师考核意见：由指导教师根据学生在实习过程中的综合表现、实际操作能力、实习报告等，结合评分标准评定；
考核等级：分为优秀、良好、中等、及格、不及格，优秀率控制在20%左右；
如果是毕业实习，则结束后须填写本表一式两份，一份存入毕业生个人档案，一份留所在学院存档与备查。

Instructions for completing the form:
1. Type of internship: cognitive internship, production internship, graduation internship, etc.
2: the column of “Assessment opinion of the internship unit”: The internship unit is required to assign special personnel to give specific guidance to the students, and this column should be filled in by the guidance personnel of the internship unit; If there is no special guidance personnel, this column shall not be filled in;
3.the column of “ Assessment opinion of the supervising teacher”: the supervising teacher will give the assessment opinion according to the students' comprehensive performance, practical operation ability and the quality of internship report in the internship process.
4. assessment grade is divided into excellent, good, medium, pass and fail, with the excellent rate controlled at about 20%.
5. If the internship is a graduation internship, students should fill in two copies of this form after the completion of the internship, and one copy of this form should be put into the personal file of the graduate, and the other copy should be kept in the college.
